*Job Summary* We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will be responsible for providing support and assistance to individuals and families, particularly in pediatric settings. This role involves assessing client needs, developing treatment plans, and coordinating care to enhance the well-being of children and their families. The Social Worker will work collaboratively with healthcare professionals, educators, and community resources to ensure comprehensive care. *Duties* * Conduct assessments of clients' needs and develop individualized treatment plans. * Provide case management services, including coordinating with healthcare providers and community resources. * Offer counseling and support to children and families dealing with various challenges. * Engage in patient care activities, ensuring the emotional and social needs of clients are met. * Collect and analyze data related to client outcomes and program effectiveness. * Collaborate with special education teams to support children with unique needs. * Maintain accurate records of client interactions, treatment plans, and progress notes. * Stay informed about medical terminology relevant to pediatric care. *Skills* * Strong background in patient care, particularly in pediatric settings. * Experience working with children in various capacities, including special education. * Proficient in data collection methods for research purposes. * Knowledge of case management practices and principles. * Familiarity with medical terminology related to healthcare services. * Excellent communication skills, both verbal and written. * Ability to work collaboratively within a multidisciplinary team environment. * Empathy, patience, and a genuine desire to help others improve their quality of life.
