As a Social Worker with VCH you will: Provide case management services and clinical and consultative social work services to individuals with complex mental health, addiction and chronic medical concerns or equivalent. Conducts biopsychosocial mental health and addictions assessments, provides counselling services, maintains client records, and attends interdisciplinary clinical/case conferences.
