Join our mission to better support seniors in the comfort of their own homes.</strong></p><p> </p><p><strong>---</strong></p><p> </p><p><strong>Key Responsibilities:</strong></p><p><strong>- Conduct wellness nurse home visits, providing 30-40 minute appointments to support stable patients managing chronic diseases in maintaining their independence.</strong></p><p><strong>- Deliver exceptional nursing care to patients in line with care plans and best practices.</strong></p><p><strong>- Act as the eyes and ears of the doctor, focusing on preventative and treatment-based care.</strong></p><p><strong>- Monitor patient progress, and document care accurately.</strong></p><p><strong>- Perform assessments of cognitive, cardiovascular, respiratory, and other systems.</strong></p><p><strong>- Address Activities of Daily Living (ADLs), including social well-being, mental health, and sleep.</strong></p><p><strong>- Educate patients and their families to promote wellness and recovery.</strong></p><p><strong>- Coordinate care on behalf of the GP, including arranging diagnostics and labs.</strong></p><p><strong>- Provide services such as wound care, vaccinations, personal care, and thorough charting by exception.</strong></p><p><strong>- Adhere to regulatory guidelines and maintain the highest standards of patient safety and confidentiality.</strong></p><p> </p><p><strong>---</strong></p><p> </p><p><strong>Skills and Qualifications:</strong></p><p><strong>- Positive, can-do attitude with excellent relational and observational skills.</strong></p><p><strong>- Proficiency in checking vital signs and conducting comprehensive assessments.</strong></p><p><strong>- Strong time management and attention to detail.</strong></p><p><strong>- Pharmacological knowledge.</strong></p><p><strong>- Experience or knowledge in community nursing, oncology nursing, or emergency nursing is a plus.</strong></p><p><strong>- Current nursing license (RN or LPN) and CPR-C certification.</strong></p><p><strong>- Graduation from an accredited nursing program.</strong></p><p><strong>- Reliable vehicle and a valid driver's license for travel to patient locations.</strong></p><p> </p><p><strong>---</strong></p><p> </p><p><strong>Compensation:</strong></p><p><strong>- For RNs: </strong></p><p><strong> - Part-Time: $55/hour</strong></p><p><strong> - Full-time $60/hour plus benefits</strong></p><p><strong>- For LPNs:</strong></p><p><strong> - Part-Time: $40/hour</strong></p><p><strong> - Full-Time $45/hour plus benefits </strong></p><p><strong>- Benefits Include:</strong></p><p><strong> - Sick leave, medical and dental, and CPP contributions.</strong></p><p> </p><p><strong>---</strong></p><p> </p><p><strong>Why Join Monarch Health Solutions Inc.?</strong></p><p><strong>- Competitive salary and comprehens- LocationVictoria, BC
