*Employment type: * Permanent, Part time or Full time (0.50-1.0 FTE, 20-40 hours per week) *Location:* This position is based out the EWPCN Medical Clinic located in Meadowlark Health and Shopping Centre with some work being performed at various community sites. *Job Posting ID: *NP-2412 *Objective: *Nurse Practitioners (NP?s) are strategically positioned in primary care to fulfill a vital role in promoting health literacy and empowering patients to take control of their health through education on disease prevention, management and self-care practices. Our NPs are committed to developing strong, long-term relationships with patients by providing patient-centered care and fostering a positive, inclusive and supportive healthcare experience. *ABOUT THE EDMONTON WEST PRIMARY CARE NETWORK:* With a mission to support family physicians in improving primary care through relationships, collaboration and innovation, Primary Care Networks (PCN) are groups of family doctors who work with other health professionals to coordinate the comprehensive delivery of primary care services for their patients. Our team of health professionals include Nurses, Dietitians, Pharmacists, Behavioral Health Consultants, Social Workers and Exercise Specialists who collaborate to provide comprehensive primary care services for the community. *In this role, you can expect to: * *Provide Direct Patient Care:* * Take part in an on-call schedule to ensure timely continuity of care for all patients * Participate in community programs as needed to deliver Primary Care services * Assess, plan, implement, and evaluate care to patients of all ages according to clinical practice guidelines and best practices * Advise patients and their families on health care including health promotion, disease, illness and accident prevention * Collaborate with members of an interdisciplinary team * Perform a patient advocacy role * Prescribe and administer medications * Perform minor surgical procedures; apply dressings, splits, slings, tensors, ear syringe; and basic wound care * Ensure accurate, objective, and timely clinical documentation in the appropriate medical record *As part of our Interdisciplinary Team: * * You are passionate about providing team-based care as part of the patient medical home model. * You want to work in a small yet mighty organization, where you are not just a number, and you have the ability to impact real change in healthcare delivery in the local community. * You are innovative by nature and want to give your input into program design and delivery in order to improve the lives of patients. * You can improve the health outcomes for a diverse patient population in the community, addressing disparities in access to care and promoting health equity. * You will find fulfillment in the meaningful relationships you build with patients and recognize the satisfaction of practicing preventative and comprehensive care. *Qualifications: * * Master?s degree in nursing * Current CRNA Registration in the Family-All Ages (FAA) stream * 5 years of nursing experience (including RN experience) in a primary care or acute care setting * Basic Life Support CPR (CPR-BLS) *What We Offer: * * Monday to Friday schedule (subject to clinic hours) * A ground floor opportunity to work with an engaged team who is motivated to contribute to evolving the role of Nurse Practitioners in Primary Care * 80% employer paid benefits, including: extended health, dental, life/disability insurance coverage and a health spending account. * Retirement Savings: 8% RRSP matching program * Paid time off, including vacation, personal, sick and professional development time * Ability to work within your full scope of practice and impact the community in a positive manner, while colla
