The Perth Family Health Team is looking to hire an experienced registered *Social Worker* to join our team!
*About Us*
The Perth Family Health Team is a new family health team funded by Ontario Health. We are a non-profit organization focused on overall healthcare and well-being of the vulnerable, marginalized, and unattached patients of our community. Our Family Health Team is built on the foundation of collaboration, empathy, and patient-centered care. Our team members respect each other?s expertise and value interdisciplinary collaboration. *Position Overview:*
The Social Worker will play an integral role within the Perth Family Health Team. The approach will focus on the person in their environment, using a psychosocial lens to assess and address factors that contribute to improving overall health.
* Support patients across the lifespan and assist them in restoring, maintaining, and enhancing their function by mobilizing strengths, building coping capacities, and linking patients to needed resources.
* Provide counselling and psychotherapy for a range of mental health concerns including anxiety, depression, grief, trauma, substance use and chronic disease management.
* Facilitate community referrals, act as consultants to other members of the Perth Family Health Team, IHP (Inter-professional Health Providers) and support patients with accessing social programs.
* Work with diverse populations including children and youth, survivors of violence, those living in poverty, and the elderly. Improve patient health outcomes through addressing behavioural elements of overall health and wellbeing including adherence to prescribed treatments.
*Key Tasks and Responsibilities*
* Assess psychosocial and ethnocultural needs of the patient and their support systems including screening for risk, determining need/eligibility for services, identifying strengths/coping capacities, and assessing informal networks of support.
* Collaborate with the team to support integrated planning and care.
* Provide counseling and psychosocial interventions for patient and family well-being, addressing mental health concerns.
* Offer resource counseling and advocate for services within complex social systems.
* Represent patients? perspectives in treatment planning.
* Assist patients and families in understanding care goals and provide crisis intervention.
* Facilitate connections to primary care providers and the allied healthcare providers services.
* Contribute to research initiatives and collect relevant data.
* Create a safe and inclusive environment that respects diversity.
* Provide psychosocial assessment and follow-up to enhance patient adherence and self-management.
* Support program planning and evaluation to meet patient-centered goals.
* This Family Health Team is newly established therefore there may be adjustments as the programs are implemented.
*Qualifications*
* Registered member of The Ontario College of Social Workers and Social Service Workers- in good standing and approved for autonomous practice.
* One or more university degrees in social work; Masters in social work, or a Doctorate in Social Work (Ontario Association of Social Workers, 2022).
* Excellent health assessment, clinical skills, analytical, and problem-solving abilities.
* Knowledge and proficiency in current, evidence-based methods and practices of primary care delivery, with an emphasis on health promotion, risk reduction and chronic disease management.
* Leadership, organization, time management, and interpersonal skills.
* Ability to prioritize, manage time effectively and be flexible in a highly active constantly changing work environment.
* Proficiency in computers and information technology is required, including electronic medical records (EMR)
* CPR certification and current driver?s license.
* A current ?Vulnerable Sector Screening Program ? Police Reference Check
