Indigenous Peoples have inequitable access to the Canadian healthcare system. The causes of the health inequities faced by Indigenous People in Canada are structural; rooted in law, policy, and institutional design ? and these structures of inequity have persisted, creating ongoing barriers in accessing healthcare services and violating trust of Indigenous people. The Indigenous population in Toronto is the largest Indigenous population in Canada, estimated to be >88,000. Approximately 1/3 Indigenous adults living in Toronto are functionally homeless and Indigenous peoples are also disproportionately represented among the larger homeless population of Toronto.
Inner City Health Associates (ICHA) is a group of approximately 200 physicians, nurses and administrative staff with a vision of a 'healthy end to homelessness' and committed to improving the health and well-being of the homeless and precariously housed communities of Toronto. ICHA staff work in shelters, drop-ins, community service agencies, encampments and clinical hub sites. Beyond direct care provision, ICHA trains future healthcare providers, conducts and facilitates research, promotes improvements in health and social service delivery, and advocates for healthy public policies and system change locally, provincially, and nationally. ICHA continues to make significant contributions to the redesign of our local system of health care for people experiencing homelessness, including those with complex support needs. *Role Description: *
Reporting to the Nurse Manager with indirect supervision from Director of Indigenous Health, the Nurse Practitioner (NP) will provide transitional and episodic primary care to Indigenous people and their families throughout the ICHA Indigenous Health Clinics/Outreach. The NP will be culturally safe to work with Indigenous communities and be able to provide comprehensive full scope primary NP care. They will be flexible, self-directed, and able to navigate health and social support systems to support patients and help them meet their needs. They will use a client-centered model with an emphasis on harm reduction, psychosocial supports, and crisis de-escalation. The NP will use their advanced knowledge, skill, and judgment to treat clients who may present with complex health conditions such as chronic diseases, infectious diseases, skin conditions and wounds, mental health and substance use issues.
The NP will work collaboratively with ICHA Indigenous Health Program?s interprofessional team of midwives, physicians, registered nurses, clinical coordinators, and community health workers to address gaps in care and health system support and navigation respecting selfdetermination, PHIPA and client consent. The NP will provide advanced health assessment of clients? physical and mental health through transitional and episodic primary care clinics to address urgent and chronic health concerns, escalating issues whenever necessary to the ICHA on-call physician or to Emergency Services.
