Under the direction of the Health Programs, Integrated Care Manager (ICM) the Indigenous Health Navigator will be responsible for developing and delivering services to community members with a focus on enhancing wholistic health. JOB SUMMARYThe Indigenous Health Navigator will provide comprehensive social work services in a collaborative healthcare model, working with multidisciplinary teams to address the unique needs of community members. They will coordinate services to assist individuals in accessing the appropriate care pathways, navigating complex health systems, and promoting healing. This role requires a deep respect for Indigenous culture and the ability to work within a framework that honours traditional practices alongside conventional healthcare. JOB DUTIES AND RESPONSIBILITIES Establish trusting, respectful relationships with patients, ensuring cultural safety and inclusivity. Advocate for the integration of traditional healing practices in care plans Collaborate with interdisciplinary teams to conduct comprehensive assessments of patients' physical, psychological, and social needs Facilitate effective communication between patients, families, and service providers to ensure coordinated care Develop, implement, and monitor integrated care plans based on strengths, needs, and goals in collaboration with patients, families, and interdisciplinary health teams Adjust care plans as necessary to address changing patient needs Provide immediate support and crisis intervention as required, assessing risk factors and implementing safety plans where necessary Advocate for patients and families in cases of suspected abuse and ensure appropriate reporting and support processes are followed Coordinate with healthcare providers and community services to ensure seamless, comprehensive care for patients Connect patients and families to appropriate community resources and educate them on available healthcare options Advocate for the needs and rights of patients, families, groups, and community within healthcare systems and broader community settings Provide educational workshops, presentations, and consultations to community members and organizations, promoting health literacy and awareness Promote understanding of social determinants of health and their impact on Indigenous communities, addressing these factors in service delivery Maintain accurate, confidential patient records and ensure timely documentation and reporting as required by OKIB policies, funders, and regulatory bodies Participate in community health needs assessments and contribute to long-term health planning for the community Engage in continuous professional development and maintain compliance with professional and legal standards, including codes of ethics and relevant legislation Participate in program reviews, quality improvement initiatives, and the accreditation process to enhance service delivery Perfor
