*Position Summary*: In keeping with the Mission, Vision and Values and to utilize Human Resources, the Social Worker (MSW) is responsible for the provision of social work services to patients and their families. The incumbent also provides feedback and direction to staff related to behavioural issues, assists with complex admission, completes psychosocial assessments of patients, participates in team functions and schedules all meetings involved with program and patients. As a member of the interdisciplinary team, the Social Worker (MSW) provides resident centered care through the provision of psycho-social assessments; individual, family and potential group counseling. Knowledge of the interaction between physical, emotional and mental health and demonstrating the skills to provide clinical care to individuals and families experiencing difficulties in these areas is essential.
*Duties and Responsibilities:*
1. Performs all functions in accordance with the Mission, Vision and Values and Administrative policies and procedures of St. Joseph?s Villa care programs.
2. Must be able to consult with physicians, nurse practitioner and other allied health professionals as part of a large multidisciplinary team.
3. Experience with the ability to triage resident needs, manage a wait list and refer to other providers and programs is required.
4. Maintain community and mental health resources and connections for residents, networking with community social workers and programs to develop partnerships that support residents.
5. Working in this environment requires health teaching to residents, students, as well as collaboration with community resources in providing services and education to patients and their families.
6. Connect family members with community resources to support their personal needs and required supports.
7. Provides goal-based counselling to residents who are cognitively capable of participating in the process.
8. Participation in weekly risk rounds, and interdisciplinary family conferences as needed.
9. Provides clinical services to residents and their families/ significant others as required or requested. This includes but is not limited to biopsychosocial assessments, counselling, problem solving and making connections for future services within community and development of resident and staff safety plans.
10. Reports concerns and complaints and participates with complaint investigations with the appropriate nurse manager.
11. Participates in Admission, Discharge and Transfer committee to review wait list referrals from HCCSS, and assists with management and coordination of bed offers where transition to LTC maybe challenging for the new resident or team.
12. Serves as a community liaison through referral(s) to alternate therapy, programs or supports in the community.
13. Facilitates meetings, reviews conferences and discharge planning with care team members, residents, families and HCCSS Care Coordinators when needed.
14. Participates in committee, in-service education and other Villa activities.
15. Participates in department team meetings and actively contribute to the development of a dynamic, knowledgeable and cohesive team.
16. Maintain and hold in strict confidence all records and information relevant to application, admission transfer and discharge, pertaining to members, families and care providers.
17. Conduct his/her professional practice in accordance with the Code of Ethics for Social Workers, approved by the C.A.S.W. Board of Directors January 1994 and remain registered with O.C.S.W. S.S.W.
18. Oversight of Social Worker (B.S.W) staff for performance and competence.
*Knowledge, Skill and Experience*
_Formal Education_
o Current Certificate of Registration with the Ontario College of Social Workers and Social Service Workers as a Social Worker in good standing
o Masters degree in Social Work from an accredited School of Social Work
o Recent (3-5 years) experience providing individual
