*Job Description: *Functions as a member of the Interdisciplinary Team under the direct supervision of the Director of Care to meet residents' care needs.
*Education Required:*
* Bachelor of Social Work
* Current registration with the Nova Scotia College of Social Workers (NSCSW)
* Minimum of one year clinical experience
*Competencies:*
* Knowledge of social work practices, ethics and techniques
* Knowledge of counseling, mediation techniques and theories
* Knowledge of interviewing skills and techniques
* Knowledge of the principles of human behavior
* Knowledge of available community programs and supports
* Ability to establish and maintain effective, cooperative working relationships with staff, families and residents
* A high level of initiative
* Effective communication both written and verbal in English with residents, families and co-workers
* Strong moral and ethics along with a commitment to Privacy and Confidentiality in all aspects of the organization
* Excellent time management and multitasking skills and the ability to prioritize tasks with minimal supervision
* Problem identification and problem resolution skills
* High degree of resourcefulness, flexibility, and adaptability
* Reliable with good attendance and cheerful disposition
* Respectful and hardworking team player
*Responsibilities:*
* Provide counseling and therapy to residents and families as appropriate; develop support groups as necessary
* Provide therapeutic crisis intervention as required
* Facilitate resident care conferences, team meetings and family meetings; participate as a member of the interdisciplinary team
* Conduct tours of the facility
* Coordinate admissions to the facility, answer inquiries from family members and general public on admission procedures
* Screen and assess applications for admission
* Assists with the admission, transfer and discharge of residents in coordination with the interdisciplinary team
* Prepares funding requests and reports relating to resident needs; maintain resident records as required
* Develop and maintain relationships with Department of Health, community institutions and community groups as necessary
* Provide in-services and education sessions to residents/families and staff on a variety of topics
* Ensure resident safety by following and applying all standard operational procedures, safe work practices and training
