*Job Overview*
Reporting to the Manager of Foster Care in accordance with the Ministry of Children, Community, and Social Service licensing requirements, MAREAS Inc. Policy, Procedure, and Practice Manual, the Case Manager?s primary role is to provide support to Foster Families and children placed in their homes.
This role will also require the Case Manager to develop a collaborative & supportive working relationship with the Foster Families and be the primary contact & liaison with Placing Agencies and other Community Partners. The Case Manager is to coordinate all administrative and clinical requirements to provide necessary care and support for each child & family on their caseload.
*Duties*
* Provide daily administrative and therapeutic support to foster children and foster families on caseload
* Liaise between Foster Parent and CAS, acting as primary Mareas contact for all CAS workers and other community partners to aid in the coordination of services and care for each child within the foster home
* Work in collaboration with CAS or placement agency workers for individual care of each child or youth
* Maintain ongoing professional relations with CAS workers and other placing agencies.
* Liaise with schools and other community partners as required in collaboration with placement agencies
* Coordinate and facilitate Plan of Care meetings as required by Ministry of Children, Community, and Social Service Standards
* Administrative duties include but are not limited to: Collection of required monthly documents for both parent and child files, Plan of Care editing and tracking, case notes, creating learning plans for each family, filing incident reports, electronic filing on the company server, and other duties as needed.
* Ensure foster families are adhering to QSF standards and the policy & procedures outlined by Mareas Inc.
* Communicate with the Manager of Foster Care and clinical team on an ongoing basis all pertinent information through weekly team meetings.
* Attend training as required to remain current in the field.
* Assist in the coordination & facilitation of monthly foster parent training
* On-call duties, including after-hours rotational schedule as required. (1 week per month)
* Meet with each child individually, a minimum of one time per month

*Skills*
* Strong verbal and written communication skills.
* Working knowledge of the Child, Youth and Family Services Act & the QSF-Quality Standard Framework outline by the Ministry of Children, Community & Social Services
* Efficient and effective computer and administration skills
* Work collaboratively and effectively as part of an integrated, multi-disciplinary clinical team.
* Ability to make decisions and problem-solve
* Ability to work independently within clearly defined policies and standards with limited supervision.
* Work effectively as part of a multi-disciplinary team.
* Ability to prioritize and organize schedule.
* Effective case noting & report writing skills.
*Qualifications*
* Degree (BSW) or Diploma in a related field, CYW, Social Service Worker, Child Development, Psychology, Sociology, and/or a combination of related field experience.
* 5+ years? experience working within or alongside the Child Welfare System and or Case Management Role
* Must have a minimum of 5yrs. experience working directly with Children/Youth & their families.
* Must have a minimum of 2 years? experience providing psychoeducation/parent support
* Professional and Proficient communication skills including phone and email.
* Excellent computer skills including familiarity with Office based software; word processing, spreadsheet and presentation applications (i.e. MS Office Suite, Excel, PowerPoint, Outlook).
