*1 Job Profile *
The CAP community worker of West Island Citizen Advocacy will share the mission, vision and values of the organization to improve the quality of life and defend the rights of all those who are living with a handicapping condition or who are disadvantaged in the community. The CAP community worker gives daily living support to residents in the Church Apartment Program. This program provides, with local churches that provide volunteer citizen advocates and other support, housing for 26 persons who have a mental illness. The CAP community worker is reviewed annually by the Executive Director and the job description will be used as part of the review.
*2 Primary Duties and Responsibilities*
*Protégé / Resident Support*
The CAP community worker will, with the resident, evaluate his needs and then provide residents with the following supports:
? Weekly apartment meetings
? Phone calls and emails
? Accompaniment to appointments (Urgent only)
? Aid with budgeting and banking
? Check cleanliness of the apartment, laundry, sheets on the bed etc.
? Healthy nutrition encouragement
? Food Pick-up
? Keep track of medications
? Keep the logs and files up to date
? Check on the general well-being and social interactions of the residents
*Volunteer Advocate Support *
The CAP community worker will support the teams of volunteers/ advocates who work with the residents to provide daily living support to them. Specific support activities are the followings:
§ Monthly follow-up team meetings
§ Training of volunteer advocates when necessary
§ Phone support
§ Other meetings if necessary
§ Accompaniment if necessary
*Liaison with other Community Resources*
The CAP community worker will be a liaison with other community resources such as the hospital, other groups, the CLSCs and the families of the residents. This liaison entails some of the following activities:
? Work together with other social workers or PACT team members to set up residents supports
? Phone calls
? Attend service plan meetings
? Attend community or Table meetings
? Participate in community activities or public education projects
*Family Support*
The CAP community worker will provide support to the residents? families with meetings, phone calls or accompaniment.
*Participation in West Island Citizen Advocacy Activities*
The CAP community worker will participate in WICA?s activities such as fundraisers, RVA, education sessions and others as determined by the Executive Director
*Record Keeping and team meetings*
The CAP community worker will be diligent in keeping up the database, the residents? files, the summaries, the interview files and the rental agreements for some of the residents. The community worker will attend weekly CAP meetings to discuss and update the residents? case.

The CAP community worker will have the following knowledge and skills:
? Computer skills (MS office suite ? Outlook, Word, Excel, PowerPoint)
? Excellent interpersonal skills
? Excellent negotiating skills
? Knowledge of the local health and social service network.
? Ability to defend the rights of the protégés in the health and social service system in an appropriate way
? Have knowledge of, and experience with, those having a mental illness and other handicaps
? Ability to attend or animate community meetings and take notes
